Why can a transaction fail?
Simulation, slippage, balances, RPC, and wallet rejection.
Common causes include insufficient SOL, slippage too tight, stale blockhash, wallet rejection, simulation errors, or temporary RPC issues.
Read the error title in the UI, check your balances, and retry when appropriate. A confirmed on-chain success is never overwritten by a later history logging failure.
